DOCTORS APPOINTMENTS
Our clinic does not have any COVID vaccines. Please book a visit with your doctor to discuss the need for any vaccines (COVID, flu, RSV, etc.) and where you can get them.

Please be advised that online appointment bookings are intended for Lakeshore Medical Group patients to book non-urgent appointments only. Phone call appointments are intended for medical issues that do not require any form of physical exam. If you would like to book an appointment that is not listed on your health care provider’s online availability, please call the office to speak with a receptionist at 905-845-4511.

*PLEASE NOTE: You are required to have a valid health card and version code upon arrival to your appointments and checking in. Please visit this website to update your health card.
**If you fail to have a valid health card at your appointment, you will need to give your credit card information to the front staff. There will be no charge to your card at this time. After your appointment, you will need to look into getting an updated health card and version code.
***You will have 2.5 months for us to receive this information from the date of your appointment. Otherwise we will charge the credit card for your visit based on the OMA billing fees.

Check-in Kiosks have been added for everyone’s convenience when you come into the office. Please head directly to the tablets to the left of reception to sign yourself in when you arrive. If the kiosk prompts you, or if you are having troubles, please check in with the front desk.
